Don't abuse this utility to slag-off the admins that banned you, or anyone that was responsible for getting you banned. We take a very dim view of that.
To file a report, you're going to need a few things...
Your Steam ID and/or Steam Profile - use this site to help.
Screenshots or Videos as evidence if you have them - You should use the Steam Community to upload screenshots.
Console Logs - Until our logging system is up and running, we cannot verify the legitimacy of sent messages unless you have reliable witnesses.

Step 1: Create a Thread in this forum. Use the Title format as follows:

If you do not use the exact title format listed above, you request will be instantly rejected, regardless of the contents or apparent urgency of the appeal.
If you do not use this title format, we will mark your report as invalid and make you do it again. This is because we...

As you can see in my imaginary list of screenshots, someone is prop-pushing other players, but the screenshots that got me banned were wrong, because I was in the wrong place at the wrong time. I did get angry - I am sorry. I may have told John Smith to f--k off and said some stuff, but I was understandably angry. As this is just a sample report, I can say as much or as little as I like here.

...and then submit. Once you've submitted your report, you will have 15 minutes to correct any formatting errors. After this time, your messages may only be edited by a moderator. You may reply to your report at any time as and when needed.
Depending on your appeal, you might get laughed out of here, you might have your ban reduced, you might be un-banned entirely, you might cause someone else to receive a ban - it's entirely decided on a case-by-case basis.
If you mess us around, use this system to insult people, or try to force us to un-ban you under duress: we will not hesitate to ban you from our forum, servers, any services we run and may be obliged to report you to our allied communities.
If you don't follow this guide or otherwise make no effort to keep your report sensible, it will be tossed in the trash. If you keep wasting our time, you will eventually find yourself unable to file reports. We're strong believers in the honour system. Be an arse to us and we'll be professional arses in return.
